Our Goals and Objectives

Our goal is to expand primary care workforce capacity to treat behavioral health conditions in seniors and low-income adults across Michigan.


Through providing clinical and programmatic expertise in implementing the Collaborative Care model (CoCM), MCCIST partners with community health centers (CHCs) as they work to implement CoCM with fidelity. In close collaboration with CHC leadership and implementation teams, we will work towards the following objectives:




Objective 1: Examine and redesign current policies, resources, and workflows to best support effective CoCM implementation.

Together with participating CHC’s, we will:

  • Conduct pre-implementation assessments
  • Provide education to the CHC surrounding CoCM implementation
  • Assess CHC “buy-in,” concerns, strengths, and challenges
  • Develop implementation strategies specific to each CHC’s needs and resources, based on findings from pre-implementation assessments
  • Review EHR capabilities and documentation templates, and modify as necessary and appropriate
  • Identify or develop a population-health focused patient registry
  • Examine and redesign current workflows, as necessary
  • Review and adapt policies as necessary and appropriate
  • Other activities as necessary and appropriate to successfully implement CoCM




Objective 2:  Develop clinical workflows and training that align with current behavioral health service provision and vision.

Together with participating CHC’s, we will:

  • Analyze patient population and needs
  • Define current array of behavioral health services and assess any gaps
  • Define clinical workflows for successful CoCM implementation, based on existing services
  • Assess CoCM treatment team comfort and knowledge with CoCM practices
  • Identify and train behavioral health care manager(s) and psychiatric consultant
  • Create individualized and standardized training plans and timelines for each CoCM treatment team members




Objective 3:  Develop a sustainability plan to ensure long-term success of the CoCM.

Together with participating CHC’s, we will:

  • Provide ongoing coaching
  • Provide additional education and consultation on specific clinical topics, as desired by clinics
  • Monitor fidelity to the CoCM model, using process and patient outcomes data
  • Optimize workflows and address barriers and challenges encountered
  • Examine payer mix and billing codes
  • Work with the State of Michigan to implement new behavioral health and CoCM billing codes
  • Develop a sustainable financial model to support CoCM
